2015 National Quality Summit, Leading Care Transitions:
Improving Quality & Safety Across the Healthcare Continuum

The 2015 National Quality Summit in Philadelphia provided participants with an in-depth look at healthcare transitions. More than 600 healthcare quality professionals attended the live and virtual sessions to connect with nationally recognized experts on the key clinical, financial and policy drivers influencing quality and safety during transitions of care. Participants went home with practical recommendations to put to work within the next 3-6 months and an overall action plan by which to engage senior leadership in improving care transitions.
