Recertification Cycle
Each recertification cycle is 2 years in length, beginning January 1 of the year following the completion of the certification examination or prior recertification cycle. The cycle ends on December 31 of the second year. During each 2-year cycle, 30 CE hours are required.
For example, if you passed the CPHQ examination in February 2009, you will be expected to renew by December 31, 2011.
A random sample of CE summaries is audited each cycle. If you are selected for audit, you’ll receive a letter explaining the procedures and how to comply. Examples of continuing education include:
- Seminar Attendance
- College Courses
- Self-Study
- Speaking Engagements
- Publications of Articles or Books
Recertify Your CPHQ Online
To complete your online CPHQ recertification, you'll need:
- an active e-mail address
- your CPHQ ID number, located on your wallet card and certificate. Please note it is not the same as your NAHQ membership number.
- your 5-digit zip code
- 30 CE hours
- your continuing education course information, including program names, dates, and hours. This service can also be used to track your CEs during your recertification cycle.
Upon meeting the recertification requirements, you will receive a new wallet card and a recertification seal to be attached to your original CPHQ certificate.
Recertify by Mail
To recertify by mail, download the recertification application and mail to the address indicated. Please allow 4–6 weeks for processing of your hardcopy recertification application.
Revocation of CPHQ Credential
If you do not recertify by the end of each recertification cycle, your certification will be revoked for failure to comply with recertification requirements. Individuals whose credentials have been revoked may not use the CPHQ credential when representing themselves. You will be required to sit for the examination to begin using the credential again.
Inactive Status
Because the field of healthcare quality is always evolving, a CPHQ must pursue ongoing education to stay current. For this reason, an “inactive” path or waiver of the CE requirements is not available.
Retirement of CPHQ Credential
CPHQs no longer employed in the healthcare quality field have the opportunity to retire the CPHQ credential. “Retired” status means that the CPHQ designation may not be used in connection with any job-related activities as an employee or consultant. Retired CPHQs do not receive certification-related mailings.
The credential may only be restored by passing the examination.
