Keep your Certified Professional in Healthcare Quality® (CPHQ) current by recertifying. Please review the Recertification Handbook prior to applying for recertification.

Recertification validates your knowledge base with the most up-to-date assessment, assures your continuing competence as a CPHQ, and maintains the standard for the profession.

* Though candidates must apply for certification online, payment can be sent by mail with the completed printed application. An additional $25 fee must be included if payment is submitted in any way other than online.

CE must be earned before December 31st and CE earned during the grace period will not be accepted.

Recertify Your CPHQ Online

To access the recertification application or to input your Continuing Education (CE) credits, visit your online recertification account. DO NOT create a new account or log-in.  All active certificants already have an account in our database. If you do not know your username or password, please DO NOT select “Create new account.”  Use the links “I forgot my username” and/or “I forgot my password.”  If you are not able to reset your login information, please call Member Services at 800.966.9392 or email This e-mail address is being protected from spambots. Once logged in, select the option that matches where you reside. In step 2, if you do not see "Apply for Recertification", DO NOT proceed.  Please call Member Services at 800.966.9392 or email This e-mail address is being protected from spambots. You need JavaScript enabled to view it .

While payment by credit card is the primary method, you may choose to make payment by check. Complete the online application, click "Print Application", and mail in with a check. Please note, there is an additional $25 fee if you are paying by check.

Recertification Cycle

The field of healthcare quality is subject to rapid and frequent changes so each recertification cycle is 2 years in length.

The cycle begins January 1 of the year following the completion of the certification examination or prior recertification cycle. The cycle ends on December 31 of the second year. During each 2-year cycle, 30 CE hours are required.

Ways to Earn CE

CE hours can come from external sources and organizations as well as from NAHQ.

Examples of continuing education include:

  • Seminar Attendance
  • College Courses
  • Online Education
  • Speaking Engagements
  • Publications of Articles or Books

A random sample of CE submissions is audited each cycle. If you are selected for audit, you’ll receive a letter explaining the procedures and how to comply. Documentation of attendance or participation in CE activites will be required if selected for audit. For addtional information on ways to earn CE, please reference the handbook.

Revocation of CPHQ Credential

If you do not recertify by the end of each recertification cycle, your certification will be revoked for failure to comply with recertification requirements. Individuals whose credentials have been revoked may not use the CPHQ credential when representing themselves. HQCC does not approve any appeals for missed deadlines. You will be required to sit for the examination to begin using the credential again.

Inactive Status

Because the field of healthcare quality is always evolving, a CPHQ must pursue ongoing education to stay current. For this reason, an “inactive” path or waiver of the CE requirements is not available.

Retirement of CPHQ Credential

The Healthcare Quality Certification Commission (HQCC) developed the CPHQ - Retired status to recognize CPHQs who have retired from their professional healthcare quality career but wish to maintain their credential. An individual wishing to obtain this designation must be a current CPHQ at the time of retirement.

An individual with CPHQ- Retired status may use the designation below their name, but not after a signature nor on a professional name badge. An individual who has been granted CPHQ-Retired status may not represent him or herself as a CPHQ. If a CPHQ-Retired re-enters into the workforce, he or she may no longer use the retired designation and can regain the CPHQ credential by meeting the current eligibility criteria, paying the examination fee, and passing the CPHQ examination. The application must be submitted during the certificants current recertification cycle.

Retirement status will not be granted once a certificants' certification has lapsed.
