How do I change my address or name?
All address or name changes must be submitted in writing by e-mail, mail, or fax to HQCC. Send your complete name, previous name or address, and your new name or address.
How do I find my CPHQ number?
Your Certified Professional in Healthcare Quality (CPHQ) number is located on both your certificate and wallet card. You can also call the HQCC Executive Office, and we can look up your number for you. This number is not the same as your NAHQ membership number.
May I apply for the CPHQ examination online?

What are the fees to sit for the CPHQ examination?

How long do I have to take the examination once I receive eligibility confirmation?
You have 90 days to sit for the examination once your application has been approved.
How long do I have to wait to re-take the examination if I do not pass?
You will have to wait 90 days from the day that you sit for the examination to become eligible to apply again.

Do you notify me when my certification is due to expire?
As a courtesy to you, recertification reminder postcards are mailed 6 months prior to your expiration date. E-mails are sent to your last known addresses prior to your expiration date. It is necessary for you to notify the office of any address changes. Ultimately, you are responsible for your recertification and your expiration date.
What is the fee for recertification?
The recertification fee is $175.
What happens if I don't recertify by my expiration date?
You have a 1-month grace period after your certification expires to submit your recertification application. If you fail to submit your recertification by that date, you will be required to sit for the examination to become CPHQ certified again.
What is accepted as continuing education?
For individual recertification, any continuing education that follows the content outline for the examination is accepted. The content outline is available in both the candidate and recertification handbooks. One hour of class time is equivalent to 1 CE credit. Please note that courses relating to clinical patient care or medical treatment are not accepted. All other guidelines for recertification are also available in the recertification handbook.
Can I send in my continuing education prior to recertifying for review?
Courses are not reviewed on an individual basis for CEUs unless the provider of the program has requested the program be reviewed for CPHQ hours by filling out the CE Approval Form and paying the appropriate fees.
